If you would like to report a possible instance of discrimination to IDHR, please complete a complaint form, also known as a Complainant Information Sheet (CIS), and submit it to IDHR via mail or email.
You may access a CIS for Housing-Related instances of discrimination here; or, access a CIS for all other (non-housing related) concerns here.

Security Measures at 555 West Monroe in Chicago: All visitors to the building are required to show a valid government-issued photo ID and pass through metal detectors; bags are searched by the Illinois State Police Protective Service Unit (PSU).
