If you would like to report a possible instance of discrimination to IDHR, please complete a complaint form, also known as a Complainant Information Sheet (CIS), and submitt it to IDHR via mail or email.

You may access a CIS for Housing-Related instances of discrimination here; or, access a CIS for all other (non-housing related) concerns here.
