Complaint Form - To File a Charge

If you believe that you have been discriminated against in one of the following areas, you need to fill out the appropriate Complainant Information Sheet (CIS) form and submit it to IDHR - via email, mail, or fax - to file a charge of discrimination with IDHR. Select the area in which you have experienced discrimination to access the CIS form and more information.

Please save the form, complete and submit via email to IDHR.Intake@illinois.gov. You may also mail your completed form to the Department's address listed on the bottom of the form.

Opt-Out Request Forms

Opt Out Request Form *(non-Housing Cases)
To request a Notice of Opt Out of IDHR's investigation and Administrative Process, and of Right to Commence an Action in Circuit Court. If the IDHR charge number includes an 'A', 'E', 'F' or 'L' the charge is cross-filed with the EEOC and the EEOC charge number is needed for the Opt Out Request Form. If you do not know (or do not have available) the cross-filed EEOC charge number, call (312) 814-4294 or email IDHR.optouts@illinois.gov. (Must be requested within 60 days of information notice).

* Parties in Housing cases do not require an Opt-Out before filing in court.

Voluntary Withdrawal Request Forms

The Department has several voluntary withdrawal request forms. The correct form depends on the type of case - whether it is Employement related, Housing related, or other.

You must submit the correct and complete withdrawal form to the Department. 

You will identify which form to use by referring to your charge number and noting the letters in the number. For example, if you charge number was 2012CA0000, you would use the "CA/SA" withdrawal form.
