Using PDF Forms

IDHR publishes and offers most online forms and web documents in a standard Portable Document Format (or "PDF" format). This broadly used format was developed by Adobe and requires Acrobat Reader or a comparable application that reads the document.

Most viewers accessing IDHR's website and forms on a PC will have the means to download and open/view these forms. 

If your computer or device will not open an IDHR form, try the steps below:

1. Right-click on the link for the online PDF form.
   
2. Select Save Link As, then select a location on your computer (like My documents or your Desktop) to save the file.
3. Go to the location where you saved the file and double click on it to open the application without needing to be online. (Adobe Reader needs to be installed and up to date)

If you do not have Adobe Reader or an applicattion to open and utilize PDF forms and documents, you can download a free version of the software from Adobe's website.*
